What is a primary care network (PCN)?

A PCN is groups of general practices working together with a range of local providers, including across primary care, community services, social care and the voluntary sector.

The aim is to offer more personalised, coordinated health and social care to their local populations.

A PCN is small enough to maintain the traditional strengths of general practice but at the same time large enough to provide resilience and support the development of integrated teams.

We have already worked together to share using the services of a pharmacist and social prescriber.

We have are also collaborating on a range of measures to share our ways of working and help continuously improve patient care and how we make the best use of our resources.

Our Aim

Health Alliance PCN’s primary business is the delivery of bespoke healthcare via community based clinics and has successfully operated since 2007. Our aim is to provide high quality patient care, enhancing patient experience by providing excellent clinical effectiveness in a safe environment.

We have a team of experienced doctors that are committed to providing you the highest standard of care and treatment.

Our reception staff are here to help you with general enquiries such as appointment booking/cancellation.

Our mission is to provide patients in Harrow with high quality, easy and convenient access to a GP when they need it.
